Payment Options

1. Payment is expected when services are rendered.

2. We accept debit cards or credit cards (Visa, Mastercard, Discover, American Express) and cash.  All cards must be signed by the owner of the card.

3. Personal checks are accepted from clients having an established payment history with us.

4. Clients needing another payment option are welcome to apply for extended-payment financing from Chase Health Advance^SM.  Applications can be completed and submitted for approval at our hospital , or you can apply online.  Highlights of the Chase Health Advance^SM program are:  

• No interest payment plans for 6, 12, 18 and 24 months. 
• Extended payment plans up to 48 months.
• Instant credit decisions
• Revolving lines of credit.

Insurance For Your Pets
